New Patient Registration
In order to register, it is helpful if you can bring us your medical card. If you do not have one, you will need to bring details of your previous GP and the address you were living at when you were registered there. We will also need your present address. (Please note, if you are a member of a college, the college is now your present address.) You will be required to complete a registration form, a new patient questionnaire and a data sharing choice form - available from reception (and from the "Registering Online" tab above).
We would encourage you to book an appointment with our practice nurse for a new patient health check within four weeks of joining the practice.
Your registration will be with the practice rather than with a particular doctor, although you will be given a 'named GP'. Dr Drury will be the named GP for patients whose surname begins with the letters A - L, and Dr Kirkaldy will be the named GP for patients whose surname begins with the letters M - Z. Named GPs have been introduced to ensure all patients have a dedicated individual who is responsible for their care. However you are always free to choose whichever doctor you would like to see.
It is important that you inform us if you change your name, address or telephone number.
Please be aware, you may be asked for proof of UK residency and entitlement to NHS Services.
