It is not possible to register online, as you must sign the three registration forms below to confirm that the details are correct before you can be registered.
However, you may print off all three forms, complete them, sign them and mail them to us, or better still, bring them in with you on your first visit to the practice.
The first form we need is the Registration form:
As part of registering we ask you to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. This is the second form that we need in order to process your registration.

Your patient record is held securely and confidentially on our practice computer system but electronic sharing of your medical record may be necessary should you require treatment in hospital or elsewhere. You must choose to give, or withold, your consent to Data Sharing by completing the choice and consent form below. Your registration cannot be processed until we receive this third form.

We now allow patients to conduct many of their dealings with us online and you can even view your own medical record online too. To register for this service you also need to complete a form. You can complete this form online and then print it, sign it and bring it with you to the practice. We also need to see two forms of ID before we can grant you online access.
